Clutch chatter, HELP ME

OK, A couple years ago I replaced the clutch in my '89 YJ and it ran fine until a couple of months ago when I heard some clicking directly related to the clutch. I would push in the clutch maybe an inch and the clicking would go away. Now the clicking has stopped but there is a major chatter, (dry pinto beans in a tin can sound) especially in gears 3 thru 5. I pulled the clutch out and found my clutch disc only wearing in certain areas, almost spotted all the way around on both sides. My retaining nut was also missing for the throw out bearing guide pin. I noticed a little bit of movement in the shaft to the transmission also.

Hopefully someone has some ideas or has had the same experience.

Warped flywheel?
 
Would a warped flywheel give the spotted wear on the clutch disc? To the question on the pilot bearing, no it wasn't changed when I did the clutch. I don't think it would be a good idea to put in the old clutch either with the wear it has on it.
